I request business counseling service from The Greenhouse. I agree to cooperate should I be selected to participate in surveys designed to evaluate The Greenhouse’s services. I understand that any information that I provide to The Greenhouse may be subject to disclosure under Florida’s public records law, subject to limited applicable exemptions. I acknowledge, understand, and agree that all information I submit to The Greenhouse will be disclosed, without any notice to me, if a public records request is made for such information, and that the City of St. Petersburg, its departments (including The Greenhouse), employees, representatives, agents, contractors, elected and appointed officials, and volunteers (collectively referred to herein as the “City”) will not be liable to me or my business for such disclosure. If I believe any information that I provide to The Greenhouse contains information that is exempt from disclosure and I wish for it to not be disclosed in response to a public records request, I will include a general description of the information and provide reference to the Florida statute or other law which I believe exempts such designated information from disclosure in the event a public records request. I acknowledge, understand, and agree that the City does not warrant or guarantee that any information designated by me as exempt from disclosure is in fact exempt. I acknowledge and understand that the City offers no opinion as to the accuracy of the reference to the Florida statute or other law by me. I am aware that the designation of information as exempt may be challenged in court by any person or entity, and I agree to defend and indemnify the City against any and all claims, demands, and actions (whether or not a lawsuit is commenced) arising out of or in connection with my designation and to hold harmless the City for any award to a plaintiff for damages, costs, and attorneys’ fees (including those of the City of St. Petersburg City Attorney’s office) incurred by the City by reason of any claim, demand, or action arising out of or related to my designation of information as exempt from disclosure.

I understand that counselors employed by, or contracted by, The Greenhouse are not permitted to recommend goods or services from sources in which they have an interest, nor are they permitted to accept fees or commissions for the sale of any goods or services that may result from The Greenhouse’s relationship with any customer.

In consideration for The Greenhouse providing me with business assistance, I agree to waive all claims against the City for any and all damages arising from any assistance provided to me as a result of my request.
